NURSE SUPERVISOR/CHARGE NURSE JOB DESCRIPTION SUMMARY: Works as a team member to provide direct personal care for nursing home residents according to established care procedures.
Works under the supervision of the registered nursing staff.
The list demonstrates the knowledge, skill, and/or ability needed to be a Nurse Supervisor/Charge Nurse.
This list is not a complete list of duties and requirements of the Nurse Supervisor/Charge Nurse.
The charge nurse manages a designated group of certified nursing employees on a nursing unit and assures smooth operations of the unit for a designated shift.
Administrative Accepts and gives shift report.
Follows "patient care assignments".
Makes rounds to evaluate status of residents and determine resource needs.
Inspects environment, identifies environmental problems, and initiates requests for correction of environmental problems.
Plans and delegates assignments.
Checks for appropriate amount and condition of equipment and supplies.
Asks for replacement or repairs of equipment if necessary and restocks supplies as needed.
Consults with Director of Nursing, Assistant Director of Nursing, or Shift Supervisor to obtain necessary resources.
Performs other duties as assigned.
Be able to perform all certified nurse aide duties as needed.
Maintains patient stability by checking vital signs and weight; recording intake and output information.
Identifies clinical and operational problems.
Obtains assistance for emergencies and assists as necessary.
Communicates unit problems and needs to Director of Nursing, Assistant Director of Nursing, or Shift Supervisor.
Advises Director of Nursing, Assistant Director of Nursing, or Shift Supervisor of topics for discussion at staff meetings.
Gives feedback to Director of Nursing, Assistant Director of Nursing, or Shift Supervisor regarding performance and evaluation of employees.
Consults Director of Nursing, Assistant Director of Nursing, or Shift Supervisor regarding the need for disciplinary action of employees; participates in disciplinary action as necessary.
Identifies, reports, and documents incidents and accidents; participates in investigation of incidents/accidents as necessary.
If employee related fills out Worker's Compensation paper work as needed.
Supports and participates in facility quality assurance programs as needed.
Assures that unit practices are consistent with regulatory requirements.
Participates on committees as necessary.
Completes records, reports, and charts in a concise and timely manner.
Clinical Participates in resident assessment and care planning activities; reviews and revises residents' assessments and care plans as needed.
Reviews, transcribes, and implements physicians orders.
Coordinates and participates in admissions, transfers, and discharges of residents.
Gives direct care as needed.
Documents and supervises others in the documentation of resident care Communicates and coordinates care with residents and their families/significant others.
Notifies residents and their families/significant others of any problems or change of condition.
Monitors and checks on nursing care activities of staff.
Counts Narcotics.
Administers medications as required per physicians' orders.
Communicates with residents' physicians.
Makes rounds to assess residents and evaluate care.
Implements, practices, and assures staff adhere to infection control measures.
Prepares staff for resident admissions, assures rooms are prepared for resident admissions.
Educative Provides instruction to staff as needed.
Participates in orientation of new employees.
Assures staff attend in-service education programs and meetings as scheduled.
Assist in planning and implementing staff development activities Attends continuing education programs to maintain competency and improve knowledge and skills.
Provides patient comfort by utilizing resources and materials; transporting patients; answering patients' call lights and requests; reporting observations of the patient to nursing shift supervisor.
Documents actions by completing forms, reports, logs, records, and activities of patients.
Maintains work operations by following policies and procedures.
Protects organization's value by keeping patient information confidential.
Serves and protects the nursing home community by adhering to professional standards, nursing home policies and procedures, federal, state, and local requirements, various accreditation standards, and respecting residents' rights.
Updates job knowledge by participating in educational opportunities; reading professional publications; participating in professional organizations; maintaining licensure.
Enhances nursing home reputation by accepting ownership for accomplishing new and differentrequests; exploring opportunities to add value to job accomplishments.

Must be physically able to provide all the services listed on a daily, routine basis.

Assures problems and complaints involving nursing services are managed in a timely and appropriate manner.
Protects the privacy of residents and employees.
Represents the facility and nursing services in professional and community activities as assigned.
Maintains positive working relationships between nursing staff and other departments.
QUALIFICATIONS: The person must meet the educational requirements of a Nurse Supervisor/Charge Nurse in accordance with the laws of this state.
The person must have current LPN or RN license in good standing.
The employee must have a current health card and be able to update as required.
ESSENTIAL FUNCTIONS: To be successful in this job, the employee must be able to stand and walk during the shift.
Movements required are bend/stoop, squat, crawl, climb, kneel, pushing/pulling, and reach above shoulder level.
Hand use is not constant and low stress.
Hand use will include occasional griping, squeezing, and using the fingers to feel and control objects.
The individual must be able to carry 30 pounds and be able to lift and move 75 pounds or more.
I must meet all qualifications and can perform all duties and responsibilities fully without additional accommodations or help.
WORK ENVIRONMENT: While doing this job the Nurse Supervisor/Charge Nurse could be exposed to infectious waste, diseases, and body fluids, which can contain AIDS and Hepatitis B viruses.
Chemical exposures are limited to cleaners, disinfectants, and personal care items.
Nursing assistants occasionally work beyond normal working hours, on weekends, and holidays.
